Hospice Advanced Practice Provider (APP)

Goshen Home Care and Hospice is hiring a Board-Certified Advanced Practice Provider to join our Hospice team. This is a part-time position. We’re looking for a passionate individual to work in a team-centered, collaborative environment. Hospice provides end of life care to patients in the hospital, home, or facility. In collaboration with the hospice medical director this 0 .5 FTE role is responsible for the direction and coordination of Hospice medical care provided by the hospice care team.

 Candidate Requirements:

Must be Board Certified in Indiana. Hospice experience preferred but not required. Exhibit strong clinical skills and the ability to work independently within the scope of practice. Communicate well. After hours call coverage to consult with hospice and home care nurses regarding patient care and symptom management as scheduled. Knowledge of regulatory and legal landscape of Hospice practices as it relates to operations.
