Adult Social Worker, PCMHT Team, Lewisham  
The PCMHT is a fast-paced front-door mental health service operated by South London and Maudsley NHS Foundation Trust with a multi-agency staff team including NHS, Lewisham Council and voluntary sector staff.  It  offers a range of short-term interventions for adults of working age experiencing mental health difficulties. The social worker will act as a Mental Health Practitioner participating in the team's duty rota they will carry out holistic assessments and develop care  plans as well as holding a small caseload  and  leading on adult safeguarding and Care Act assessments within the multi-disciplinary team.
