Job Summary

Summary
Manage and coordinate the activities of the liaison team under the supervision of and in collaboration with the senior leadership. Coordinate the referral process to ensure a smooth transition of patients to a Spaulding Rehab Nursing (SRN) facility. Assure patient meets criteria for admission, is clinically appropriate, and is ready to transfer to a post-acute provider. Perform preadmission patient evaluations when required. Work with SRN facilities, MGB clinical operations, and referring facilities to remove barriers to create a smooth and timely transition of care. Support census management to ensure admission budget targets are met. Develop and maintain collaborative relationships with internal staff and external referral sources. Communicate with referral sources and resolve customer concerns with positive outcomes.

Essential Functions
-Manage and mentor a team of admissions staff, providing guidance, training, and support to ensure optimal performance and adherence to established procedures.
-Ensure trends or barriers to achieving established admission and referral targets are identified and collaborate with admitting directors and senior directors to remove those barriers, including insurance and regulatory-related issues.
-Coordinate referral process and ensure appropriate escalations are managed to ensure a smooth transition of patients to SRN in accordance with the liaison process and established timeframes.
-Validate that patient assessment information is obtained and audited from a variety of sources and data is transmitted in appropriate systems (4Next/Epic and other applications).
-Contribute to collaborative relationships being developed and maintained with internal and external referral sources to ensure and maintain customer satisfaction. Ensure that the admissions process complies with healthcare regulations, privacy standards, and facility policies, including adherence to the Health Insurance Portability and Accountability Act (HIPAA).
-Responsible for ensuring that patients are accepted, confirmed, and qualified as described in the liaison process.
-Accountable for pertinent information from SRN being communicated to the current medical provider to meet patient and provider needs.
-MGB CC referral influences are identified and communicated to the manager for proper follow-up per established guidelines.
-Ensure payor mix results are achieved according to established targets.


 

Qualifications

Education
Bachelor's Degree Healthcare Administration required or Master's Degree Healthcare Administration preferred

Licensed Practical Nurse [LPN - State License] OR Physical Therapist [State License] - OR Physical Therapy Assistant [State License] - OR Registered Nurse [RN - State License] -OR Occupational Therapist [State License] -OR Occupational Therapy Assistant and Aide [State License] - OR Speech-language Pathologist [State License] - OR Licensed Independent Clinical Social Worker [LICSW - State License]

Experience
Experience in healthcare admissions or a related role, with demonstrated experience in team management and process improvement. 2-3 years required

Knowledge, Skills and Abilities
- Knowledge of IRF, LTAC, SNF and level of care regulations.
- Background knowledge/work with a clinically complex patient population.
- Broad knowledge of various insurances and government reimbursements.
- Highly developed interpersonal and relationship-building skills, working with a diverse population.
- Must be able to organize and prioritize effectively while working independently and adapting to changing conditions.
- Effectively solve problems while meeting demands of all parties.
- Be comfortable with and enjoy working in a fast-paced environment.
- Be able to make appropriate clinical decision based on information provided independently.
- Computer proficiency required. Microsoft Office applications are preferred, with the ability to learn new software.
- Effective verbal, electronic and written communications skills using the English language.
- Broad knowledge of various insurances and government reimbursements is desirable.
- Highly developed communications and interpersonal skills, working with diverse population.
- Work independently, be self-directed and contribute as a member of a team.
- Anticipates challenges and develops and implements strategies for addressing them.
- High level of service delivery. Demonstrate initiative with ability to prioritize work, meet deadlines and adapt to changing situations.
