Job DutiesResponsible for providing patient evaluations and assessments to ensure the pre-admission process results in the appropriate level of care set by the patient placement criteria of ASAM.
Responsible for providing the biopsychosocial evaluation and assessment to determine appropriate level of care using ASAM level of care index and diagnostic tools.Identifies appropriate discharge planning to patients after intake and evaluation process and refers to appropriate level of care.Supplies pertinent information gathered in clinical assessments to multidisciplinary treatment team.Serves as initial contact person for referrals.Completes all pre-certification reviews with managed care companies identifying clinical diagnosis, level of care and ASAM and DSM IV criteria, and initial problems that need to be addressed along with tentative discharge date.Assists in maintaining current admission and discharge census as needed.Responsible for evaluating and assessing family dynamics.Begins to involve family with treatment program.Gathers preadmission intake information, verifies insurance information and compiles chart to begin medical record.Completes all releases of information, informs patients of their rights and patient confidentiality.Coordinates payment arrangements with insurance company or financial agreement with patient or responsible parties.
