Schedule: Monday–Friday, 8:30 AM–5:00 PM

Local: $57/hour
Position Overview
Amergis Staffing is seeking an experienced RN Case Manager for a 13-week contract assignment with an Addiction Services program in Lynn, MA. This role is essential in supporting patients with substance use disorders by coordinating comprehensive care, developing individualized care plans, and ensuring seamless access to medical, social, and behavioral health resources.
The facility is specifically looking for a self-starter who communicates well, can work independently, and has a strong interest or background in patient care and care management.
Key Responsibilities
+ Establish contact with patients appropriate for RN Case Management based on diagnosis, treatment plan, risk factors, compliance, ER utilization, or grant-based criteria
+ Collaborate with PCPs, Team RNs, Clinical Pharmacists, Providers, patients, and families to develop and revise individualized care plans
+ Provide education and support related to medication management, diet, exercise, specialty referrals, equipment use, immunizations, and self-management goals
+ Coordinate care with internal departments and community agencies (e.g., Social Services, WIC, HAWC, Behavioral Health, Dental, Eye, Pharmacy)
+ Ensure timely scheduling of follow-ups, labs, and needed visits; conduct outreach calls and follow up on no-shows
+ Document all interventions using learning assessments, teaching guidelines, plans of care, and the EHR/EPM system
+ Support patients in maintaining their health goals through ongoing education, check-ins, and resource coordination
Requirements
+ Active Registered Nurse (RN) license in Massachusetts (must be in hand)
+ Previous experience in Community Health or Nurse Case Management preferred
+ Bilingual in Spanish, Khmer, or Russian beneficial
+ Strong attendance history and positive work attitude
+ Medical clearance, BLS, and COVID-19 vaccination required
+ Preferred: CCM certification, experience in substance use treatment programs, and knowledge of community resources
