Activity Coordinator
Position Summary The primary purpose is to plan, develop, organize, implement, evaluate, and direct the engagement programs for the long term care residents. Provides supervision and direction to the Program Assistants. Minimum Qualifications Work Experience Previous experience in a social or recreational program facilitating activities for individuals or experience in a long term care or adult day program preferred. Supervision experienced preferred.
Education Bachelor’s degree in Recreation or a related field or two years of experience in a social or recreational program in a health care setting within the last 5 years, one of which was full time in a therapeutic activities program or has completed a training course approved by the State of Kentucky for Activity Professionals.
Certifications/Licensure Certified or eligible Therapeutic Recreations Specialist; OR Certified or eligible through the National Certification of Activity Professionals; OR Currently licensed as an Occupational Therapist in the Commonwealth of Kentucky; OR Currently licensed as a Certified Occupational Therapy Assistant in the Commonwealth of Kentucky. Valid Kentucky driver’s license required. A motor vehicle record check will be performed annually. Job Specific Performance Standards The duties listed below are a summary of the major essential functions of this position.   Regularly plans, develops, and evaluates activity program objectives for the resident’s engagement. Reviews applicable JCAHO standards and other regulatory standards to ensure standards are being met. Keeps abreast of departmental and corporate activities, projects, and ventures. Implements effective Memory Care Program. Completes admission activities assessment and RAP’s on newly admitted residents within mandated time periods as required by standards or regulatory requirements.  Updates and reassess as mandated. Participates in weekly interdisciplinary care plan meetings and addresses engagement activity needs. Maintains daily participation records and quarterly progress notes for residents. Visits residents and carries out individualized engagement activity plans. Assists residents in securing items necessary for individual activity projects. Helps transport residents to and from activities. Prepares Quarterly Vehicle Reports. Plans and implements seasonal decorations for long term care areas. Schedules, plans, and posts monthly activities calendar visible to residents, staff, and families. Posts individual activity calendar in resident rooms. Coordinates and facilitates Pet Therapy. Communicates with cognitively impaired, aphasic or other residents with impaired communication according to the plan of care. Leads and facilitates group activities. Facilitates monthly Resident Council Meeting (or designee). Directs and supervises volunteers assisting with activities, Program Assistants, and CNA staff which may assist with activities. Maintains records relating to activity department, including assessments, progress notes, and resident activity participation records as mandated by standards or regulatory requirements to remain in compliance. Completes required documentation for Resident Council Meetings.  Facilitates that follow-up is reported back to the next resident council meeting. Operates company vehicle.  Adheres to driving laws and regulations and assures safe operation of vehicle.  Conducts general maintenance checks on company vehicle.  Reports maintenance issues.  Maintains record of maintenance checks.
