At Conviva, we’re seeking innovative people who want to make positive changes in their lives, the lives of our patients, and the healthcare industry as a whole. **Must be fully Bilingual in English and Spanish** **This position requires travel between 2 Centers has needed** Conviva is seeking a candidate to join our Team as an **Access Representative** , who is interested in being part of a team that focuses on excellence and service to others. **Location Schedule - Required to work out of the following Centers throughout the week:** **Conviva Care Quail Roost 3 days out of the week** 18623 South Dixie Highway Miami, FL 33157 **Conviva West Kendall 2 day out of the week** 15516 SW 88th St, Miami, FL 33196 **Conviva Care Centers as needed or coverage** **Essential Functions:** + Increase Dual Eligible membership in Conviva locations + Assist patients with applying for services available through DCF such has Medicaid, and Food stamps. + Provide any extra assistance available through government programs that patient could qualify for. + Assist centers with increasing patient retention, patient loyalty, and growth + Serves as an advocate and educator to applicants in need of assistance of services provided by DCF. + Travels/works out of multiple Conviva centers. + Comply with all HIPPA and rules and regulations. + Attention to detail when documenting on EMR/ECW + Provide weekly and monthly report to direct supervisor. + Contribute to team effort by accomplishing related results as needed **Required Qualifications** : + Bilingual English and Spanish + Knowledge of Microsoft Word, Excel and Outlook + Administration support experience in a healthcare industry + Strong customer service skills + Attention to detail + Must be willing to travel locally to work out of multiple Conviva centers. + Effective verbal and listening skills + Ability to multi-task, prioritize and manage time effectively. + Use time effectively in order to screen maximum amount of patients as possible. **Preferred Qualifications:** + Experience working with DCF and/or the DCF ACCESS program + Experience resourcing assistance programs to help low-income households and/or better the lives of others. + Social Worker or case management experience + Microsoft Access proficiency **Use your skills to make an impact** **Additional Information:** **Monday through Friday: 8am to 5pm** This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $39,000 - $49,400 per year This job is eligible for a bonus incentive plan.
