In this position, you will facilitate hospital transfers and direct admissions, determine daily bed resources and oversee bed management logistics for the health system. This position is vital in gathering accurate data to enhance efficiency and promote patient safety and need prioritization. **A caregiver in this position works 11:00am to 11:00pm, about 48 hours in a 6 week period.** A caregiver who excels in this role will: + Serve as the primary point of contact for telephone patients with a variety of health or illness questions. + Utilize effective and professional communication, clinical knowledge and nursing assessment skills to quickly triage patient status. + Provide intake, assessment and triage for internal and external hospital transfers and direct admissions on request in accordance with standard operating procedures. + Elicit information from referring providers, review patient clinical information and assess patient status for appropriate placement. + Collaborate with health care providers and hospital leadership to optimize patient outcomes utilizing clinical expertise and incorporating nursing and management experience with critical thinking skills and business intelligence tools to achieve optimal outcomes both clinically and financially for the patients and the organization. + Apply solid understanding of the complexities of the patient status and the process and capabilities of each system hospital to seamlessly facilitate and achieve enterprise bed placement. + Provide oversight of all bed management functions. + Function at any point in the patient flow process designated to ensure the safe and efficient placement of patients and effective utilization of resources. + Demonstrate cost effective management of enterprise bed resources to enable the organization to provide quality care to patients.
