$85,000 | Wound Care-Patient Care Navigator| Madisonville KY

Salary: $50 / Hour
Patient Navigator:
+ Travel from facility to facility developing relationships with the patients and the facilities to coordinate patient discharges
+ This is essentially a sales job / liaison with the facility
+ 3-5 years of experience with discharge planning at a hospital or skilled nursing facility – strongly preferred
+ No license required
+ Should have a Bachelors in Social Work, Psych, Nursing, etc.
+ Interview process
+ 2 interviews with the client – First with Lea the coordinator, then with the regional manager
+ Pay Structure:
+ $85k base salary
+ Up to $65k annual bonus based on conversion rates of discharges to patients
+ Full time employee eliogible for benefits and start accruing PTO after 90 days.
+ Spread:
+ $9k over 13 weeks ($692/wk)
