Patient Navigator:

Travel from facility to facility developing relationships with the patients and the facilities to coordinate patient discharges This is essentially a sales job / liaison with the facility 3-5 years of experience with discharge planning at a hospital or skilled nursing facility – strongly preferred No license required Should have a Bachelors in Social Work, Psych, Nursing, etc. Interview process 2 interviews with the client – First with Lea the coordinator, then with the regional manager Pay Structure: $85k base salary Up to $65k annual bonus based on conversion rates of discharges to patients Full time employee eliogible for benefits and start accruing PTO after 90 days. Spread: $9k over 13 weeks ($692/wk)
